The only required form that cannot be done electronically is the ANNUAL PHYSICAL and MEDICAL HISTORY Form. Parents will be able to print the physical and medical history form in the electronic forms section. The physical and medical history Form must be completed by a parent and physician. Once this is completed, High School students should turn their physical and medical history into the athletic trainers. Middle School students should turn their completed physical and medical history into their respective athletic coordinators.

Electronic Forms:

1) There are 4 electronic form tabs that you will be required to view and complete.

2) Parent / Guardian and Student must be present when completing the forms and have the Student’s School ID available.

3) Forms will not be electronically accepted without all required information completed and signed at the required locations.

4) Do not leave a required location blank Type N/A, or None if you do not have the answer.

5) To sign a document, click inside the signature box and hold your mouse down, this will allow you to create an “electronic signature”. If you make a mistake and need to start over, click on the refresh icon next to the signature box. If you are using a smart phone or IPAD, hold your finger to the screen and sign.

6) You will receive a confirmation email once the document(s) have been reviewed and accepted by Sheldon ISD.
